Patients’ Responsibilities
As your healthcare partner, we ask that you:
General Consent to Care
I hereby consent to care and treatment as deemed necessary and proper. If the patient is a minor under the age of 18, parent or legal guardian must accept this agreement.
Billing Policy
It is our policy that patients pay for all services at the time of the appointment. A 48-hour notice of a cancelled appointment is required, otherwise you will be charged for the missed appointment. We will do our best to fill the cancellation on shorter notice, but if it is not filled you will be charged. It is the patient’s responsibility to file insurance claims to their carrier. Upon request, a universal insurance form (a superbill) will be provided along with all the necessary information so that you may submit a claim directly to your carrier. Credit cards, personal checks, and cash are accepted. Returned checks will incur a $25.00 fee.
Financial Responsibility
I understand that I am financially responsible to Waves of Health, and I agree to pay for all charges rendered at time of service.
Insurance
Waves of Health does not contract directly with any insurance plans. Superbills can be provided.
